Welcome to Novatio School – a fully online, ESA-funded private school for students in grades 4-8. Our innovative approach to education provides students with high-quality virtual learning in a supportive environment tailored to individual learning needs. Novatio operates on a full-time school schedule, Monday through Friday, from 8:00 AM to 2:00 PM Arizona time. While we offer personalized instruction and engagement, daily attendance and active participation during these hours are required.

This application form will collect information about your child, your ESA funding status, and learning needs. The sections in this form include:

  • Student Information – Basic details about your child
  • Parent/Guardian Information – Contact details for the person submitting this form
  • ESA Funding – How your child’s tuition will be covered
  • Virtual Learning Readiness – Technology requirements for online learning
  • Parent Agreement & Next Steps – Key acknowledgments before submission
  • Document Upload – Required files for enrollment

Please submit one application per child. If you are enrolling multiple students, complete a separate form for each.
